Please look over all of this information and be sure that an athlete's medical history/physical are on file with the Briar Cliff Athletic Department before he/she can represent BCU.  Also, every BCU student-athlete must read the insurance information and follow-up with Gallagher Koster.  Completed forms can given to the student-athlete's coach or be mailed to:
